The Fecal Calprotectin test measures the level of calprotectin, a protein released by white blood cells, in stool. It is used to detect intestinal inflammation and differentiate between inflammatory bowel disease (IBD) and non-inflammatory conditions like irritable bowel syndrome (IBS).

Fecal Calprotectin (Stool) Test Overview

Calprotectin is a protein found in neutrophils (a type of white blood cell).

When the intestines are inflamed, neutrophils migrate to the gut and release calprotectin, which is excreted in stool.

Clinical uses include:

Detecting inflammatory bowel disease (IBD) such as Crohn’s disease and ulcerative colitis.

Differentiating IBD from IBS, since IBS does not cause inflammation.

Monitoring disease activity and treatment response in IBD patients.

Assessing unexplained gastrointestinal symptoms like chronic diarrhea, abdominal pain, or blood in stool.

The test is performed using a stool sample.

Analyte: Calprotectin protein concentration in stool.

Normal Range: < 50 µg/g stool (varies slightly by lab).

Elevated Levels:

50–200 µg/g: Borderline, may require repeat testing.

> 200 µg/g: Suggests significant intestinal inflammation (IBD, infection, or other pathology).

Clinical Use:

Detects intestinal inflammation.

Differentiates inflammatory vs. functional bowel disorders.

Monitors disease progression and treatment effectiveness in IBD.
